For me it's:

  • Low res textures in a game that sports high res character models, it defeats the point.
  • Low render distance.
  • Streaming textures.
  • Megatextures (seriously not even close to being perfected yet).
  • Chromatic Aberration.
  • Jaggies.
  • Screen tearing.
  • Input lag
  • Sometimes DoF.
  • Sometimes motion blur.
  • Poor Anti aliasing options.
  • Capped fps.

On the technical side of things...

1) screen tearing
2 frame rate slowdown (don't mind a few dips - would have to be a slideshow to bother me)
3) long loading times

On the gameplay side of things...

1) QTEs
2) interactive cinematics
3) lack of offline/local play

Cutscenes that you can't skip. That are purposely done. To pad out the game time. Mainly put in a boss fight, that will get repeated a lot. They do bloom, DOF, and motion blur. They don't work like real life, at all.
