Konami probably wanted to "The Last Guardian" us until they announced the next Silent Hill, but there were just too many people working on it outside Konami to pull it off.

My understanding is that sandwiches had been going missing for months. Kojima was actually too busy to notice any of this and then simply for unlucky one day when he accidentally grabbed the wrong one. Now they think he's been behind it all from the start.
