I beat him first time but I was out of heals (27 vials and 1 Iosefska's Blood) and had just 25% of my HP left.

Theres another boss after him if you get the 3rd ending which I did.

First game I've played from the developer and I enjoy it a lot. I think I'm at the end of the game I just need to chalice dungeons before i finish it.

I would have finished by now but I'm trying to beat all the optional bosses on my first try before finishing it. And since I'm allowed to use the TV at night only, it's taking me a little bit longer than I thought.

I don't know if you plan to plat the game but if you had chosen "refuse" you would have gotten a new weapon right at the start on your 2nd playthrough.
