Ninja Gaiden 3 Razors Edge, at least for the Wii U. I can't remember if No More Heroes 2 was censored in Europe or not.

Manhunt 2 would be the winner although I've never played it so I guess Turok or Resident Evil 4.
Signature goes here!
Other: Manhunt 2 (Uncut).
You have to have a modded Wii to play the game properly. Uncut the game deserved the 94% score that Ngamer gave it in their exclusive uncut review. One of the best games developed last generation, and it was completely ruined by the BBFC and ESRB.
Madworld or Ninja Gaiden 3

There's a bunch out there, like Killer7, Resident Evil 4, Manhunt 2, House of the Dead (Like 2, 3 and Overkill), Madworld and some more. Maybe Eternal Darkness also counts?
If I have to pick one, I'll go with Manhunt 2. Motion controls actually added inmmersion. It felt gratifying executing those bastards >:]
Just started to play Splinter Cell: Blacklist on Wii U. Though it certainly isn't the most macabre game I've played so far, it's the most brutal I've played on a Nintendo console in quite some time now.
