I'd go with Manhunt 2. Games like No More Heroes and Madworld, while violent, are almost comical with their over-the-top mayhem. Manhunt is more gritty and disturbing.

It's similar to comparing the Crazy 88 fight in Kill Bill to something like Hostel.

Call of Duty World at War, Black Ops, and Black Ops II.

Torture scenes galore; people getting their throats slit, their kneecaps shot out, lit cigarettes jabbed in their eye, writhing in agony as they are burned alive, having glass stuck in their mouth and then punched so that it breaks, being shot in the throat and choking on their own blood with a sickening gurgling sound, having their eye cut out...

Nobody delivered gore to Nintendo platforms as unflinchingly and seriously as Treyarch.
