Metal Gear Solid V: The Phantom Pain, or Final Fantasy XV. I'll go with the latter.

I believe technologies have been advancing technologies allow blurays to hold even more space. I think they will be using triple layer before going to two discs. I don't know if there's going to be a game that does, but if there is, I don't think it's something we've heard of yet.

Not counting games like MGS Legacy Collection which shipped with 2 discs last gen, FF, (or any other big JRPG for that matter) or Gran Turismo will probably be the first ones.

Why are multiple discs so terrible? I mean, it's certainly not ideal, but a game or two on multiple discs has never been a problem...(?)

Something near the end of the gen. If Rockstar releases 2 GTAs this gen, perhaps the second one might do it.

Disk swapping would be so 5th, 6th, 7th gen. Disc swapping would be a taste of nostalgia. Instead of swapping CDs or DVDs, finally get to swap Blu Ray disc to play a game.

Square Enix then?

On 5th gen this was more common. Every Final Fantasy released had at least 3 cds. Resident Evil 2 and Metal Gear Solid had 2 cds. But none could beat Phantasmagoria and its seven cds.

Because PS3 fans kept saying X360 was inferior because some games had to change DVDs... useless debate, unless in case we had too much compression to keep both plat with 1 discs and that impaired PS3 version (not usual)... kowen is just taking a jab at PS fans.

Due to modern h.264 & h.265 compression techniques and dual layer blue ray technology, there will be no need to use two blue ray discs ever. 4k movie resolution may be the only reason for multiple blue rays. Or maybe some day lossless video.

Blue rays are also quite expensive compared to classic CD or DVD technology.
