I beat him first time but I was out of heals (27 vials and 1 Iosefska's Blood) and had just 25% of my HP left.

Theres another boss after him if you get the 3rd ending which I did.

Holy shit that's a lot of heals you stacked from the runes. Around 30 vials worth used in 1 fight is insane.

I'm at the worm boss thingy on the water. I cheated on that hunter in the building. I kept doing a sneak attack then ran up the ladder. :) I'm out of town working so I get to play it again till friday.:(
