Mostly hard nowdays but if I think that may be a problem I go ahead on Normal first.

Normal then hard if I really like the game.

default/normal for most, if i like it enough to replay i'll play it on hard

I'd say usually medium, unless I'm going for achievements/trophies then I'll do hard.

I usually play on normal, although I started Resident Evil 5 on veteran.

Normal-Hard difficulty, sometimes I set the difficulty to Easy to get the feel of the gameplay controls

I think the better question is "What games do you wish had a higher difficulty level". Zelda TP and Okami comes to mind.

depends if I've ever played a game like it then normal, if I've played A quite similar game( or if in a series I've played a game) then hard but since mario kart wii was my first mario kart( yeah yeah really late i know) i played automatic( but i switched to manual fast)
