For me mostly medium first then give it a scorch on hard.

Altough I sometimes try hard first and then regret it.

Depends on achievements/trophies. More recently I have been going for hard for that very reason

Normal. If I wanna hurry up through the game easy. I get no satisfaction or fun from playing a game in hard.

I start on default, and if the game merrits a second playthrough then the Hardest difficulty

I do the hardest difficulty available. After a while if i completly just utterly suck at the game then I'll lower it down one difficulty.

Games I had to do this with:

Champions of norrath ( didn't realize you were suppose to play through a low difficulty and then load your leveled character for a higher one)

Street Fighter games ( i just plain out suck at street fighter)

Ninja Gaiden Black
