Scene in Spec Ops the line when you bombard a certain enemy camp with some mustard or sulphuric acid and melt everybody. It completely caught me by surprise and I have never felt so much remorse and regret.

Roth covering Lara while she traversed underneath a bridge. Also, sniping cultists with the bow at night, predator-style.

Tomb Raider 2013 had some really amazing set-pieces. A few too many, in fact, as there's never a quiet moment in the gameplay to digest what's going on. There were also two ZEROES too many human enemies, but I digress. Those set-pieces are absolutely incredible, even if the rest of the game was faulty.
