Vader being Luke's dad is really obvious to an adult watching that film. I mean they continually hint at it in the first and second film with the way everyone reacts when he asks about his dad and how they're all worried he'll end up like him, especially in Empire.
Also Shiek being Zelda is blatantly obvious and I only played all the way through that game fairly recently (like 2011 I think).
Aerith dying is a big one as that's a seriously surprising moment

I think the one that caught me off guard was Baten Kaitos twist, I actually did not see it coming.

Yeah, I'm glad I got to experience Final Fantasy VII before Internet access and gaming sites became ubiquitous. That said, sometimes seeing spoilers in games actually gets me even more interested because I want to see the context in which they happened.
Sheik I did guess before the reveal.
I always wish I'd been old enough to have seen The Empire Strikes Back in 1980 just so I could be an asshole and walk out of the theater past the lines saying loudly, "Wow, who would have thought Vader was Luke's father! Boy, I never saw that coming!" That idea (and kind of the idea for this thread) came from that one episode of The Simpsons where Homer did that. But this thread has turned out some pretty good discussion. :)

I mean I'm not old enough to have figured it out as an adult because it's so part of the pop culture lexicon now, but watching them as an adult they REALLY hint strongly at it throughout the second movie especially

The thing that always got me, honestly, was the love triangle they seemed to be building between Luke, Leia, and Han early on in the first couple of movies, only to reveal near the end of Return of the Jedi that Luke and Leia were sibs. I guess since the romance was kept on a G-rated level it wasn't exactly icky, but more like an "oooooh-kay" thing.

well yeah the makeout thing in Empire is pretty creepy looking back, but I don't think Lucas had thought of that twist at that point. Then again "there is another" was in Empire so who knows. Maybe he thought of it later in filming and didn't feel like taking out the kiss to keep people thrown off
