Someone could spend years contributing to the site, make tens of thousands of posts, and be permabanned for making a dozen or so bad comments.

I think you need to be quite the douche, ignore warning and break the rules in order to get permaban. Don't feel sorry.

Have permanent bans ever/can be ever overturned?

Are requested bans ever overturned?

Personally, as someone who understands online addiction and worked in the gambling industry, I would certainly like to see an absolute policy on requests and never be overturned.
