Super Metroid, OoT, Castlevania Chronicles, Super Punch Out and Symphony of the Night, once a year, but I can easily beat them in 1 sitting after playing them uncountable times.
Doom 1, about 3 times per year. Doom 2 once a year. I can definitely beat them in 1 sitting but it is tough not to get frustrated and take a break, especially D2. It is ridiculous hard on middle difficulty and any above.
I played the Last of Us twice in a row but probably won't play it again. Same with Uncharted 2, Lords of Shadow and Max Payne 3.
I have played FF2, 7, 8, 10, Metroid, Doom3, Wolfenstein original and Return to, many Zeldas, some Nintendo games, Half Life 1 and 2 multiple times but not in many years.

Pretty often. Replaying Dragon's Dogma right now. It's all I think about. So much backlog on every console but I find myself running back to games like Street Fighter, Metal Gear series, Gears of War series, Arkham series, etc. The greats just keep me coming back. I'm really wanting to start a Mass Effect trilogy playthrough. Sometimes I have to stop myself to play the newer stuff, though.

If it's a good game, 2-3 times.
I've played through my favourite game, Resident Evil 4, eleven times.
I've also played each star in Mario Galaxy 1 & 2 and each mission in games like Goldeneye Wii and the Wii COD games 5 to 10 times.
If I'm going to plonk down $60-$100 AUD for a game, I make sure I get my money's worth.
Depends on how much I like the game. Bioshock, Tales Of Vesperia, The World Ends With You, Final Fantasy 6 and a few others I played several times.

Not often, but if it's a really good game I'll always come back to it every year or so.

I have a few games I replay about once every year.
Betrayal at Krondor, Baldur's Gate 1 and 2 and perhaps Thief or Deus Ex.
Never really unless a trophy compels me to or I have to break in a game for a new console that was on an old console.
I only play games excessively. Game is released, I blast through it in a week usually at least 10h per day. That's why I often take a week vacation for my most anticipated games. After that I'm pretty much done with it. The stronger it is story focused the less likely I'm going to replay it. For example TLOU was one of the greatest games I ever played. I will most likely never touch it again. ( unless it gets a definitive edition on PS4)
