I tend to use default names unless I can think of a really cool/funny nickname.
For example in Pokemon, I named my Machamp Roids. My Forretress Walnut.
The X legendary shall be names Shishigami and Y legendary the Y-vern.
EDIT:Oh you mean the Avatar name. I always call myself Butthead.

If the game provides a character name, I will almost always use it. Otherwise, I use a different name for each game.
If I remember correctly, I used Basque for World of Warcraft, Darius for Elder Scrolls IV, and Ulysses for Wizardry: Tale of the Forsaken Land. I usually linger on the name selection screen in RPGs for at least five minutes trying to imagine the perfect name :P
Oh, and I always use Link for The Legend of Zelda :)
I always use Busted, It all started 6 years ago when i played an mmo for the first time and had a hard time naming my character, it just occurred to me because i saw a guy named Bu$, i didn't want to copy him so i just imagined that word (w/o knowing it was an actual word in another language) and that was my name, a friend that played the same game and went to school w me started calling me Busted in real life instead of using my real name and soon everyone else followed so i became ''Busted'', i loved those times (like, first high school years) so that word means a lot to me, in fact all of the guys i'm still in contact with keep calling me like that, i'll always be Busted haha and i use it for every single game.
Well I have a few. I used to always do Cloud until a few years ago. Then I moved to Cid. Since Demon's Souls, I've been using Atlus because it's both an awesome game company and a bad-ass Greek God.

Depends on how much personality the dev team gave the main character. If it's a game like Pokemon, Zelda or Chrono Trigger, where the main character is pretty much a soulless avatar with no real outstanding character traits, then I use my own name to really help me get into the role and imagine myself as part of the game. If I'm playing as a person who is definitely given his or her own personality, backstory and makes his or her own decisions throughout the game without my choice or interaction, I usually go with the default name, because then I feel more like a third party observer instead of that specific person.
If there isn't a default name, I have several that I often choose from. Tek, Magnel, Gradets, i think are my most frequent.
4 ≈ One
i've created over 150 characters, so i typically reuse those names based on the type of game it is, and the characters abilities. i usually tend to keep the names as is cause the characters usually don't reflect the abilities of my own.
