You must use the blade mode and cut the enemy where the red square tells you to, that's where the blue thing is (I call them that because I don't really know what they are, they look like blue intestines xD). The person playing the demo does it nearly the beginning:

I believe Kamiya brought the S rank to DMC. He's been using it for quite a while. His influence has rank in hack n slash titles, especially at Platinum.

X and A at the same time on Xbox 360, you can choose a direction with the analog stick if you want.
