link was teleported to holodrum and labryna by touching the triforce, they are probably paralell worlds. koholint is an island and not that big, as we never get to see and ocean in hyrule(and no, wind waker doesn't count) we are lead to think hyrule is more of a continent or a bug country that anything else

 

Koholint was a dream world.
