Normal is the default setting and what I start on, but if you're playing a Clover/Platinum games it starts on VERY EASY which isn't easy lol and then progresses to get harder and harder the more you beat it like Viewtiful Joe... @_@

Depending on the game/genre either Easy or Medium, usually medium. Then my 2nd playthrough on Hard.

MGS was on Normal (Western).
Most FPS' on Normal.
Uncharted I had on Easy but I'm playing through on Hard now and I haven't died once.
Beat em' ups usually on Easy.

Easy or Normal.
I don't have the time to screw around and play certain parts over and over again.
If I really really like the game then I also try harder difficulties (Halo on Legendary, CoD on Veteran and so on)

Well I usually play on normal first and if is it seems to easy in the first few hours i'll restart in a harder difficulty.

On shooters I usually play on the hardest difficulty possible and if it seems impossible I'll work my way down.
