Actually, we don't have this middle name crap here where I live.
It's your first name, then your family name (can be either your father's or your mother's or sometimes even both).
My name is Leandro (my first name) Johann (my mother's family name) Silva (the name my mother took when she married my father. It was either this or Chagas. Pity Chagas didn't win).

I don't have one. In my country, everyone just has the first name and (usualy) one last name (fathers, and in rare cases, like divorce and some additional stuff, mothers).
