Hail to the Chimp is probably the most current one
well,
I think the biggest disasters (financially and for the image of a developer/a console/the console industry) are more interesting.
E.T. ofcourse would be a very strong contender for the top of that list.

Shenmue would be another one. Every DC owner would have to buy it twice for it to break even, right?

I don't know how well they sold, but games like Jurassic Park Trespasser and Diakatana were critical disasters and should be seen as reasons why you shouldn't get too exited about games based on previews and hype.
Wii Sports, seeing as though it hasn't actually sold on it's own merit.
Inb4 ban.
MGS4!

too bad, i was looking forward to this game cause I love Hellboy.
I might still buy one when it drops to $5.99 in a month or so.

By numbers it would most likely be
Sucessful Console:
1990 Nintendo World Championships: Gold Edition (Nintnedo NES)
Copies Produced: 26
So it sold 26 copies!
Not so sucessful Console:
Kizuna Encounter (PAL Neo Geo)
Copies Produced: Less than 12
So it sold 12 copies?!
Both games are now valued at $10,000 +!
But obviously these games wouldn't count, since the producer made limited copies!
