Ragnarok Online. Its version of guild warfare is still the most fun I've had in an MMO. Tried a failed game called Horizons for a bit, tried Lineage 2 for a bit, tried one of those super-pretty Korean MMOs for a bit. Played WoW for a few years, amazing game. Played the new BioWare Star Wars MMO for several months--it's the best story-centric experience I've had in an MMO.
Tried to go back to Ragnarok, actually, but they deleted my accounts. I was filthy rich and I wasn't going to start over from scratch.

Runescape. Which was THE shit back in the day.
Some free trashy 2D MMO back in ~ 2004 or 2005. It was so bad that I cannot remember its name :|
Vote the Mayor for Mayor!
I don't remember which was first... I played Shadowbane, Everquest, Anarchy Online, Asheron's Call, WWII Online, SWG and FFXI before WoW... I know there were others but I can't remember them at the moment.
Whichever of those was earliest.
*edit* I'm seeing runescape mentioned a lot, does that count? It might be my earliest then... For some reason I never really counted it among the other mmo's.
Not sure, possibly Ragnarok Online
