I think it's both myself. Weak optical reading devices that make themselves more prevelant to dustyness in launch consoles.
Aka, unless your house is a real mess... it's likely due to the drive. That likely can be fixed by their "cleaning solution". Aka some sort of layer of something that likely got missed... and if it was bad enough you probably needed a new reader.
That was my theory as soon as i heard you had to send in your machine for a cleaning. Then again that's what NES had wanted you to do even up to the NES days.
