IMVHO it's mainly matter of point 3, extremely bad marketing, including, amongst other things, an extremely bad transition from Wii to Wii U, with Ninty practically killing the former too early together with the very Wii brand it wanted to reuse for the successive console, pure genius, and this also helped already bad communications to foster the misunderstanding that Wii U was just a new version of Wii. Basically a major marketing mistake (killing Wii too early) and a major marketing flaw (communicating ineptly) that worked together to achieve the maximum synergy possible.
