You're right I confused the terms. And Vivian is indeed a ghost, so I guess this doesn't really matter :P

Depends.  If appearance makes it impossible to avoid knowing he/she is/was a man, then he/she would only be a woman to herself.  Depending on how you look at things.

If Vivian identifies as a lady, she is a lady.
