Ohh, I'll second Pandemonium, not sure how many times I completed it but I played it a LOT. What a gem.

There was a time when the only game I owned was Super Mario Bros. for the Nes. I bet I beat that game somewhere in the neighborhood of 100 times. We used to play two player and bet on who would beat the game first. I've even purchased multiple versions of the game over the years and still beat it on a regular basis.

I've also beaten games like Zelda LttP (got my time down to three hours, at one point, I think), Super Metroid (I think I got my time down to less than 1.5 hours if memory serves), Resident Evil 1, and Chrono Trigger (beat New Game plus so many times, my characters were level **).

In recent times, that title probably goes to Metal Gear Solid 3 or Resident Evil 4.

This gen? Probably Tomb Raider Legend (which I also bought on PS1 and PSP) or Gears of War 1--both on the 360.

Crazy but I've played through the old shitty Xbox game, Bulletwitch somewhere in the neighborhood of four times, trying to unlock an Achievement for playing the game 15 hours. That goddam Achievement hasn't popped, yet!!  I'm tempted to just leave the Xbox on for a day just to get this Achievement.  Makes no sense!

Lifetime....... Streets of Rage 2 (200+ times) 140+ times on the Megadrive, 60+ and counting co-op games on XBLA.

This Gen...... Ninja Gaiden 2 (11 times) acolyte, warrior and mentor difficulties, then another 8 playthroughs to get each of the weapon achievements

Super Mario Bros. (NES): more than 20 times.
Ocarina of Time (N64): 4 times

I rarely re-play games...

Metal Gear Solid for the PSOne, like 30 times.

It's not my favorite game (but I really like it), but Luigi's Mansion is #1 with about 16 playthroughs. I had 12 next to the star literally only 1 yr after it came out. Short game ftw!

Second is my favorite game ever (Paper Mario), with about 12. I ran out of save slots, so I had to delete some as I played. And this was before I rebought the game.

After that, it is hard. OoT was around 6, Bully (idk but I LOVE that game) around 5, Sunshine around 4-6, PM2 around 4-5. Ratchet and Clank games around 4 each. I've played virtually every single game I owned at least 3 times.
