I'm going to guess you have a problem with your HDD. This is just a theory, but you could have a corrupt sector and when you installed the first time it installed over that sector. You then re-installed which moved it to a different location.
You could always pull the HDD and hook it up to a PC and run some diagnostic tools. (so long as they used FAT32 for the file system)
