MGS4 has to win. Very logical reasoning:

MGS4 beat GTA4 in the Action Adventure category. GTA4 is gone.

Now, the platform awards:

PS3, with an inferior Fallout 3- MGS4
PC, with no MGS4 and an inferior GTA4- Fallout 3
X360, with a very good Fallout 3 and no MGS4- GTA4.

So, GTA4 beat Fallout 3, MGS4 beat GTA4. Nothing has beaten MGS4, so MGS4 will win Game of The Year. Same with IGN.
