Either this guy takes estrogen (the pill) in order to get more feminine or he takes it through another way involuntarily. Maybe he was fat once this can lead to some more than usual breast tissue too. If you have puffy nippels or you feel some hard substance under your nippels you have some breast tissue. Normally a mens breast should be soft fat or hard muscle or a combination of both. Its actually pretty normal for guys to have it and is a result of the high hormone dose of the puberty and aromatization of testosterone to estrogen.

Also steroids have this as a sideeffect. And males do cosmetic surgery in order to fix this. 

But hypogonadism can be a cause too.

To be perfectly honest I think that he wants to be a girl and is talking the pill many people do that.  But its just a theory we will never know.
