Worst decisions:

1.  Everything Atari did after the 2600.  The games, the 5200, the 7800, the Jaguar, all of the controllers, everything.

2.  Virtual Boy - 100% bad idea

3.  Sega rushing everying.  The SegaCD, 32X, and Saturn were all crappy.   The poor Dreamcast had no chance.

4.  Too expensive with mediocre games -> Intellivision, 3D0, CD-i, and Apple Pippen I'm looking at you.  PS3 may be joining you soon.  I'm going to give Neo Geo a pass here as there were some great games for it.

5.  Cartridges?  In the age of CDs.  N64 you gotta be kidding me.  The Gamecube discs weren't great either.

I forgot about Virtual Boy! O man what a bad system that I own and love because of the great games on it. I absolutley adore Red Alert and the Wario game on it. Some of the greatest games ever created. And nobody can say the Gameboy Micro was a bad idea because it cost Nintendo like ten dollars to manufacture the system and then sell it for 100, 1 sale = ten paid off, plus it took shelf space away from PSP during it's launch. So I'm supremly confident they made a little bit, at least broke even.

Nintendo and "We can't live without Square", the Cartridges cost to the devs, the Virtual Boy...

Sony, well the cell, "this is not a game machine, it play games, but you don't need a computer anymore", the whole 50% higher than the competition ,for the devs and the buyers... and the cost of the PS3...

And Microsoft, well, the RRoD... Bad choice of manufacturers

I wouldn't presume to call it the worst, but just about everything Sega did between the Genesis and the Dreamcast has to rank pretty high on the list.

i loved the virtual boy.... god i waned one so bad..... sigh

I think Nintendo and Sony were both crazy when they backstabbed each other back and forth when they almost made the Satanic PS1/SNES hybrid. But still, how fucked would the industry be if that actually happened? Worst hypothetical monopoly ever.

Worst mistake is when you rush games to completion while hyping it as the best ever, without having it fully done. No names needed we have all played one or two or ten of them.

Instead of 3DO, I think the Atari Jaguar 64 was horrible. It was expensive, had the worst controller ever, like 1 or 2 good games, and Atari was considered a joke before it even came out.
