There are so many but heres my list.

Man did the creators super metroid did number on me with the music. The swampy caverns in maridia and second cavern of brinstar scared the living crap out of me.

The sounds of the bosses and metroids made when they were attacked in the first game scared me.

The background music of in bottum of the well and shadow temple in OOT were really creepy too.

Her voice brings fear to my heart...

also the was a level in Max Payne that was disturbing to say the least watch at 2:47

I kept dyeing and dyeing because it was hard and the controls were stiff and hearing that baby cry over and over and over was driving me crazy.

DUDE! Remember there was something on a cell making noises, but it was invisible? That gave me the creeps back then because I didn't knew where it was coming from.
