Resident Evil GC (I think it was the remake of the original on PS1). The part where you are walking along the long corridor with black and white tiles and a dog jumps in through the window is the scariest thing I have ever experienced.

Resident Evil on the PSX scared me, too. Okay I was a bit younger and it was the first survival-game I've experienced
