I imagine most people don't have it because it's not a game for everyone.
I hadn't planned on getting it because I've never liked any other MGS game but my friends convinced me to go to the midnight launch so I decided to pick up a copy since I wanted something to play on my PS3. I played a few hours of it, realized I still don't like the games, and finally got around to selling it last week for Dead Space (for the PS3 even-- loving the game by the way).
Anyways, where was I?
Oh, yeah, not everyone like MGS4. It's a niche game.
you know i havent even picked up mgs4 yet, mostly because i havent finished the 3rd one. and i kinda forget what happened in the first and 2nd. but i heard its a good game even if you havent played any of the others, anyone confirm this? but yeah ill be getting it for sure once i get some extra cash from the coming holidays

As long as you know who Liquid, Snake, Big Boss and to a certain extent Raiden and Otacon are, you'll be fine.
I loved it.
But the story is hard to follow... It's confusing. That's a huge factor. When I was playing snake eater. I really didn't know what the fuck was going on story-wise. I had to look it up to get up to date.
Overall, it's not a genre that alot of people get into easilly.
Not their cup of tea I guess?
ive got MGS4 and im going to give my honsest feeling about it.
Firstly ive got to say i really enjoyed it despite not playing any other MGS game. I thought their should have been half the cut-scences as there was too much info to take in. The gamplay was epic, well certainly the last quater of the game. I never got bored of actual gameplay as their was only 3 hours worth, for me on easy anyway. And nearly 3 hours of cut scenes. I would reccemend this game to people who are patient. The online play, well to put it fairly it was shit. I mean there were only people in the roof tops and as soon as you got up they killed you, and it takes about 100 bullets to kill someone.
Overall i would give this game a 7/10

There was a lot more than 3 hours of gameplay, even on easy. I played first time on Naked Normal, there was a good 7 hours of gameplay and 7 hours of cutscene.
Metal Gear Online was pretty disappointing, but not really bad. The reason it took you 100 bullets is because people have more health than they do on Liquid Easy. You have to aim for the head and use first person shooting.
Like I said, some people just seem to like this kind of game, others don't.

A lot of PS3 owners are casual future LBP gamers, so I would not expect them to be interested in MGS4 
Giant mechs... world controlled since the 1800s by a group of select individuals and now by a computer system... everything is controlled and soldiers are injected with micro computers that regulate their ability to feel emotion... that's a bit over the top for most people... those people buy Splinter Cell instead if they buy any stealth action game.
