I still have to decide if it's my favourite (anyway it ranks very high), but when someone writes "intro to a videogame", the first that springs to my mind is Fallout 2. Curiously, being a very far away and nebulous memory, the second that I often remember is Ghosts 'n' Goblins. Third comes Syndicate Wars.

Seriously. No More Heroes FTW? The intro was so damn good it had me grinning in front of my monitor for the next hour.
Also, I liked the REmake intro, as well as Eternal Darkness, Resident Evil 4 and SSBM.
