There are exceptions, though. Mass Effect, for example, has already set the avatar surname, so it's weird to have him called Wright Shepard (doesn't sound bad either). In these cases, I use Kyle for men and Erica for women.
There are some other obscure examples. Bloodborne has such setting that I felt compelled to be more creative. My first character was called Dorian Wright, and the second one, Amadeus Nox.
Whenever I'm set to make a very evil character, I like to call them Jack; I roleplayed some time ago in a Angel vs Demon forum, and some user had a character by that name, and he was the most sadistic monster of the bunch. It kinda left an impression on me, and I like to honor him by giving his name to my avatar. Most notable examples, my Fallout 3 Evil Karma PJ, or my unethical Grey Warden in Dragon Age.
My pawn in Dragon's Dogma is called Syra, because it reminds me of a close friend. She used that name when roleplaying.
If I have to imput a complete name, and the game encourages to use my own, like in Metal Gear Solid 2 and 5, I use Damián Wright. My real name plus my favourite surname of all time. (it's kinda my artistic full name at this point).
There are more, but those are the ones I can think of right now.
I usually go with Manon.
It's a girls name, but to me it sounds vaguely male enough to be a unisex name. And it has this slightly exotic sound about it that makes it work well in a lot of diffrent universes.
Let's see. I usually don't like making characters like myself, that includes names. So I never use my real name or my online names. Also one of the reasons I play female roles in many character creator RPGs (even FP ones). I'm playing out that characters life, it's not my life.
Anyway, I have a standard few I use in every game. My main female name I got from a nude model... won't mention what it is... you'll just google it. Second name I use is 'Keeta' or 'Peeta', they are made up.
For males I use Nathaniel, then Kedan (latter being made up).
Hmm, pie.
It depends on how many characters the game will allow me to have, but I will usually go with Platina regardless of gender

I always choose names that have a V as the first letter. Main character is always Viv. I believe that's unisex enough for the horrible instanced that I have to choose a male character. Everyone after that is based off that. Vivelle, Vivor, Viva, Vive, Vivette, Vivenne... you get the gist.
I also never rename characters that have default game names. I did that once and it was horrible.
