Believe me, that's the most frustrating part of making a puzzle level. Hours of revising trying to get stuff to stay put. Every block in my latest level is strategically placed to slow you down enough to allow the Clown car to catch up (so it doesn't disappear), be visible where possible, and not have essential items reset while you play. I almost gave up on Companion Bill cause the damn launcher kept disappearing. It's only a few blocks off screen when things reset, and more annoyingly some things go / fall through obstacles off-screen, eg blue platforms and black flowers.
And yeah you have to take Yoshi along, use a door or pipe and everything you're not holding goes back to its starting place. Only bomb damage, removed coins and used power ups are permanent. (Which has its own problems, not being able to reset that)
It's pretty obvious the engine was purely designed to be used for always going forward in one direction.
