Maybe not the highest, but the Overmind from Starcraft deserves an honourable mention. Kill several plants worth of intelligent life forms should but him in the rankings.

Klaus/Zanza from Xenoblade Chronicles. The guy accidentaly destroyed the fucking universe, killing all living beings except him and mayneth. And after that, he willingly killed thousands/hundreds of thousands.
