Hasn't happened with PSN or Steam and is highly unlikely that it will happen. Even so, if you're on PC you can always use an alternative cloud provider and sync saved games folders with that particular service (e.g. google drive). With that said, fact is, you still have a copy in two locations; one in the cloud and one on HDD. If one goes down, the other is still available. If your memory card save is corrupted, no backup.
And if your account gets hacked, there are procedures in place for you to get your account and saved games back (even if the person who has hacked your account has deleted them as the company in charge will likely have older backups). If someone steals your memory card(s), that's that. All the data is gone.
