Isn't there two different teams for Halo in 343... one for MCC and one for 5? They've shown to learn their mistakes in MCC through constant feedback, updates, and free content including a game. I heard Halo 5's beta looked good and played well in terms of match making and so forth. So I doubt Halo 5 will have those same issues.

What? Why would you think that? It even got delayed already for further polish.
I'm going with The Division. Big new IP made by Ubisoft with already an intense hype. A graphical downgrade from what we've seen is pretty much a given, and knowing Ubisoft recently the game will be rushed and thus broken at launch. I'm fully expecting The Division to not only be broken, but also to be the biggest disappointmet in 2015 based on the game's current hype, which is already dying due to Ubisoft's recent releases.
Other good contenders are games with yearly installments with a set launch window such as Sport games, Call of Duty, Assassin's Creed etc.
