I could care less of what other countries think of ours. All that tells me is the rest of the world is full of ignorant people.
Overweight (most circumstances) and smoking are choices.
Poor is different. It's difficult to simply make a vacuous statement about this one. Most of the time, it's a product of the environment, lack of opportunities, lack of assistance, which are overbearing and inhibit that drive to become bourgeois.
I say smoking is tops.
Poor is something all countries have, and doesn't single America out really.
Smoking again is a worldwide thing, and isn't some centralized theme here,
Over weight is a growing issue here, and is always associated with the US.
Gonna go with Obesity here.
Most of the reasons you listed for thanking the poor people seem like things that are hampering our development.
Much like the slaves of the Roman republic alleviated the need for innovation the poor do so now.
This is the Game of Thrones
Where you either win
or you DIE
Fat people are like the only group of people that are ok to make fun of anymore!
Well... that and stupid people.
Which is kind of weird.
Obesity is the worst problem... largely because it could be fixed if we just simply changed our culture and attitudes a bit.
It actually doesn't take a lot of effort or money to make healthy foods.
It actually takes less money to eat healthy then it does unhealthy, the problem is... it takes a small amount of effort. While unhealthy food is just sitting their prepackaged or ready to be picked up in a matter of minutes.

Huh? What are you talking about?
Smoking? USA is rather considered to be one of the countries with the harshest anti-smoker laws in the world.
Living off the government? USA is rather considered to have a third world social system where poor/unemployed people can hardly expect anything from the government and many end up homeless.
The obesity part is correct though - While USA has actually meanwhile fallen to place 2 in worldwide obesity, most people will probably still first think of americans when they think of overweight people.

* OCED Obesity.
Worldwide Obesity the US is quite farter down the list due to countries like Nauru.
