I don't see deviations as a bad thing, as long as they are followed up with a mainline entry in the franchise.  If not, then this is the worst.

After that, I think that the one you called "inbreeding" is the worst for the franchise.  Increasingly focusing on a single niche (usually vocal) audience of your product will lose you customers in the long run.  It would be great for customers in that niche though.

It's not really tiny upgrades but too much recycling within the same gen bothers me. Forza 2 -> Forza 3, Shift -> Shift 2, TDU -> TDU2

Sequels that continue the story that are more then a year apart, HL2 episodes, Mass effect. Don't leave me waiting too long, I don't remember what I did in the previous game anymore.

Sequels that tell a new story, but play exactly the same, that are not far enough apart, AC 2 -> AC brotherhood.
