I would have to say the mile high club achievement on COD Modern warfare was hard as sh-t. When I got it I did the rodeo run around the living room.

The second attempts and the third and the fourth are even harder, because the difficulty increases with each new game , i.e playthrough. And not just by considering your levels, it becomes harder regardless, even if you are lv. 100 on your second playthrough its harder than the first.

Alma, from Ninja Gaiden on the Xbox.

In general Ninja Gaiden was one of the few games that could beat the shit out of me, and still make me come back for more.  Alma was the hardest of the bosses IMO and she was just shy of being cheap.  I wanted to break controllers, fighting her was so frustrating and there was a honest sense of relief AND accomplishment when I finally beat her.

Good times.  HAHA.

Sonic 2 Final Boss Fight

Level 3 (Turbo Tunnel): Navigate obstacles like pits, walls, hovering walls at ever incresing speed. You miss you die!

Level 4 (Arctic Caverns): Run through slippery corridor and platforms over spike with snowballs thrown at you. Hit the spikes, you die!

Level 6 (Karanth's Lair): Get from point A to point B riding on giant snakes but don't fall off or you'll die.

Level 7 (Volkmire's Inferno): Fly a plane through fire, electrical current. Touch them and you die.

Level 9 (Terra Tubes): Pass through the tubes while fighting hammersharks you throw yu in spikes and rubber ducks with quite the bite.

Level 10: (Rat Race): Race with a rat to kick a bomb. Miss a turn and you're the one exploding

Level 11 (Clinger Wingers): You're chased by a giant ball while on a unicycle, get every turn right and you'll make it... of course you have to fight to ball when you're done.

Level 12 (The Revolution): Climb up a huge tower, fall off you die!
