Well if they weren't as concerned with making the shell as small as possible, then they wouldn't be so loud. Sure, it saves PS money in many ways, and people are fixated on small/thin electronics, but most of those devices aren't designed with such performance in mind. OG PS3, was pretty big, and loud, but it didn't just have PS3 hardware inside. OG PS4 was too small and too loud. It should have been the size of Pro, which would have allowed for a larger fan and/or heatsink.
I hope that PS5 ends up much quieter, even if it's as big as the XB1 was. Now they would have to keep the power supply inside the shell, and design the console so it doesn't look like a boring rectangular VCR. If XB1 had the brick inside the shell, and was more visually appealing on the outside, like the XB1S, then I think the size wouldn't have mattered as much. PS console design has always been original, so I wouldn't worry about it being ugly.

PS4 - $- We must build a console that’s affordable, charges for services, and pumps out exclusives.
PRO -%-We must build a console that's VR ready, checkerboard upscales, and sells but a fraction of the money printer.
PS5 - ^ -We must build a console that’s a generational cross product, with RT lighting, and price hiking.
PRO -&- We must build a console that Super Res upscales and continues the cost increases.
