Darkadia is without a doubt, the best free game collection tracker out there. The only thing, it doesn't have the most extensive database. It'll cover you for 99% of everything else though.
It also fills the other 3 requirements. I have a link to mine in my signature.
Backloggery is cool for it's completionist aspect, but it's by far the most tedious to use.
