You have almost no swearing or violence in the anime, heart attacks aren't considered as violence. The most violent scenes are when Light and L punch/kick each other...

I mean, a guy going out of his way murder people in whatever way he likes seems pretty violent to me. Heart attacks weren't the only way people were killed.

For something that covers such dark material, it's not surprising to me for it to include darker things.
