People sat at their desk and thought about what would make a fun game that nobody has played yet. Then the publisher would figure out how to market the game.
Nowadays, a game is often based around the success of somebody else's game. Then they have to think about demographics and appealing to particular markets. If you ever wondered why so many games look the same and are marketed the same, this is because the creative process has shifted from the developers to the marketing executives.
