Itagaki just isn't a very good designer. He had some good ideas in Ninja Gaiden, but they were flawed (as shown by how much better the Sigma versions were due to a few minor tweaks). I'm sure he used to be really good, but he's lost his way and put too much stock in jiggle physics.
