To start it you need to beat Amelia and touch Laurence's skull so it turns to night. Then go back to Hunter's Dream and there will be items waiting for you. Afterwards go back to Cathedral Ward, take a left, and let the Amygdala on the right grab you.

Hmm, I only have 40 dollar on my PSN and wanted to buy The Witcher 3 Expansion Pass for 25 dollar.
