Report Missing Games & Cover Art in the Database Here

Hey, as VGC's newest Game Database Admin, I am working on getting the database up to date. Please list any games which are:

-Missing from the database entirely
-Have no cover art
-Have incorrect publisher data
-Have incorrect release dates
-Cancelled Games

all here, and I will fix them. Please, if possible provide the information (publisher/release date) which is missing. I will fix everything in this thread as I read it, and I will report what I have fixed as I go. I'm going to try to get this thread stickied (if any mods read this please sticky this thread so it's always on top).
