there is mansized vent in Halo 3 campaign, it just happens to be that i got glitched up there when i played with selnor once on legendary, he was down there getting his ass handed to him and i was up there getting my ass glitched 
(couldn't move at all until i throw a grenade at myself and respawned)
also another game is Syphon Filter , they're so huge that you can actually do a crouching walk on them in the old games ,while in the newer games you have to be on all fours
Sly Games also used them a lot but that isn't realistic yes?
