Some games really use it to their advantage, like God of War. If all the blood turned to fairy dust, the atmosphere of the game would be ruined.
Some games, like 50 Cent: Blood on the Sand, are just a mockery of everything an M-rated game is meant to be.
Though ESRB gets a little ridiculous and throws Ms at everything. Most 15s in the UK are M in NA.
