Different culters follow different rules. In the US waitresses are paid under minimum wage, and the tips are meant to boost their salery above minimum wage. If you refuse to tip and they make less than minimum wage, then the company is obligated to pay them minimum wage. So not tipping does not hurt the waitress, but may cause the food prices to go up anyways (if enough people didn't tip).
In other countries, no one tips and the waitress actually has a decent salery, so no big deal. If you go out of the US, you should check what the local customs are so you either don't look like an ass, or get robbed.
Also, it used to be the standard that 10% was ok, 15% was good service, now 10% is bad, 15% is ok, and 20% is good. Some people have even gone on to 20% average. Personally, I tip tops $10 no matter what the bill. They are waiters and have 10 tables and I only sat there for 1 hour. $10 from 10 tables over 1 hour is a lot more than I make per hour.
